To Him alone take refuge with all your Being, oh Bhārata. By His grace you shall attain supreme peace, the eternal state.

Practical conclusion: Tam eva śaraṇam gaccha — to Him alone go as refuge.

Sarva-bhāvena — with all your Being (mind, intellect, emotion, action). Tat-prasādāt — by His grace. Parāṃ śāntim — supreme peace. Śāśvatam sthānam — eternal state, permanent. Śaraṇāgati (total surrender) is the path.
